Our certified Geriatric Care Managers assist older adults and persons with disabilities in the Charleston, South Carolina area to maintain their maximum functional abilities. We strive to respect the autonomy of the individual and deliver care-coordination and support services with sensitivity to preserve the dignity and respect of each individual. In addition, our GCMs are experienced guides and resources for families of older adults and others with chronic needs.
