Care Manager
- Provides consultation, assessment, care planning, care implementation, quality assurance, medical advocacy, education and empowerment for the consumer and supportive counseling to clients in their own home setting
- Must have excellent customer service skills, be able to view the person as a holistic entity and love working in a team and with care recipients and families
- Must have extensive supervised experience in working with older adults, persons with disabilities and families
- Must have excellent knowledge of community services and benefit programs
