Geriatric Care Management What is it? Who Needs it?

Four steps ensure you and your loved one receive the personalized care you need and deserve:

Assessment
An experienced Geriatric Care Manager will conduct an in-person interview with you and your family to discuss and assess health problems, functional abilities, safety, resources, strengths and desires. Input from all involved parties is included.

Planning
Services are tailored to alleviate problems and meet the needs of the client. Of course, the personal care plan is always discussed with the client and the client’s representative. Services may include resources from the community such as adult day care, home delivered meals, medical consultation, and emergency response systems.

Monitoring
Our Care Managers provide regular monitoring visits to (1) oversee home care services, (2) continually re-evaluate the individual's needs (3) update the care plan as necessary and, (4) monitor medications. Continuing personal contact allows us to identify and address potential problems before they become a crisis.

Care Management especially benefits those responsible for the care of a dependent adult but unable to manage and supervise home care due to time constraints or geographic distance.
