A health assessment may be needed when your senior has multiple problems and issues that need to be evaluated and addressed. One of our Nurse Care Managers personally visits your loved one and assesses the physical and mental health, activities of daily living, living conditions, adequacy of medical care, unaddressed health issues, and other miscellaneous care needs.

Based on information from the visit and on information from the physician and other involved parties, a full report is written which details the problems and needs. This document is often used by the family’s physicians, attorney, trust officers or conservator as a care-planning tool serving you and your loved ones.
