Hospital to Home Care Concierge Services

Services include:

1. Transportation home from hospital if requested
2. Home visit by Geriatric Care Manager (GCM) to identify a personal care plan for the caregiver to follow also includes a snapshot safety assessment
3. Light Housekeeping
4. Meal preparation
5. Personal care needs, eg assistance with dressing, toileting and stand-by-support
6. Medication reminders
7. Nurse on call during the 8-hour shift

1. Hospital Case Manager and Care For Life discuss case
2. Discharge notes are faxed to Care For Life
3. Time of discharge is closely estimated
4. Transportation needs are identified
5. Key contact person for patient is notified by Care For Life to advise them of time the actual 8-hour shift begins
6. Geriatric Care Manager meets caregiver and patient at home to set up personal care plan and assess home for safety issues
7. On Call Nurse is available during the 8-hour shift to answer any issues or concerns
8. Faxed confirmation of services provided is faxed back to Hospital Case Manager after 8-hour assignment is completed
